New Patient Checklist

Before your first appointment with our experts, please ensure you have the following items ready when you visit:

• Co-payment, as needed through insurance
• Your insurance card or full insurance info
• Legal photo ID
• Referral form or paperwork from referring provider, as needed through insurance

Please wear loose-fitting clothing without any metal pieces, like metal buttons or zippers.
